Overview
Medical acupuncture is the practice of acupuncture by a licensed physician, grounded in contemporary neuroscience rather than traditional Chinese medicine theory — though the two overlap substantially in practice. Dr. Migdale earned her Contemporary Medical Acupuncture certification at McMaster University in Ontario, Canada (2024), one of the most rigorous and respected physician acupuncture training programs in North America. She also holds VA Battlefield Acupuncture certification (November 2025). As a fellowship-trained pain physician with deep roots in manual therapy, Dr. Migdale integrates medical acupuncture as a powerful and versatile tool within her broader pain management approach.
The Neuroscience of Acupuncture
Medical acupuncture works through well-characterized neurophysiological mechanisms. Needle insertion at specific anatomical points activates peripheral nociceptors and mechanoreceptors, triggering local and segmental effects including release of endorphins, modulation of substance P and calcitonin gene-related peptide (CGRP), and activation of descending pain inhibitory pathways. The result is a local analgesic and anti-inflammatory effect, combined with central pain modulation — explaining acupuncture's effectiveness across a broad range of pain conditions.
What Medical Acupuncture Treats
Dr. Migdale uses medical acupuncture for: myofascial pain and trigger points (where needle deactivation of trigger points is a primary mechanism); chronic neck and back pain; headache and migraine prevention; neuropathic pain; post-surgical pain; osteoarthritis; and as an adjunct to other interventional treatments. The evidence base for acupuncture in chronic pain is among the strongest of any complementary medicine modality — with high-quality meta-analyses supporting its effectiveness for musculoskeletal and neuropathic pain conditions.
Battlefield Acupuncture
Battlefield Acupuncture (BFA) is a specific auricular (ear) acupuncture protocol developed by Dr. Richard Niemtzow for rapid pain relief in military settings. It uses semi-permanent ASP (Aiguille Semi-Permanente) needles placed at five specific ear points corresponding to pain regulation pathways in the auricle. BFA has been widely adopted by the VA and military health systems for acute and chronic pain management. Dr. Migdale completed VA BFA certification in November 2025.
